When Earl finally wakes up from his coma, he believes in Karma again and considers Billie his soul mate, but he doesn't know where to find her, so he asks her ex-boyfriend Frank for help.

When Clark wonders if the world would be better off if he never came to Smallville, Jor-El shows him what life would have been like if he had never existed.

The Survivor Medical Team looks at the cut on James's finger and tells him if it doesn't get better by the morning, he'll need surgery. Then the next morning, Alexis takes a bad fall and cuts her knee.



At the Reward Challenge, the castaway meet with their family members and compete for a trip to Jellyfish Lake which is won by Alexis. She picks Cirie and Natalie to go with them on the trip. After the challenge, the medical team checks James's finger again. Medical decides that James must leave the game to go to the hospital. Meanwhile, on Exile Island, Amanda finds all 3 clues and discovers that the hidden immunity idol is hidden back at Dabu camp.



Alexis returns from Jellyfish Lake and is shocked about James but worried about her own injury. Cirie states that because of the all-girl alliance at camp, they must beat Erik at the next Immunity Challenge. The next morning, Parvati says she would not be part of voting out Amanda no matter what the circumstance was.



After Erik wins the Immunity Challenge, Amanda and Parvati think Natalie for the vote while Erik, Alexis and Cirie think Amanda should go. Before Tribal Council, Amanda realizes she is the next to go and searches for the immunity idol under the camp flag.

Renee becomes convinced that Betty is in love with Daniel and thus starts a rivalry between the two for his affections. In the meantime, Gina returns to Queens to flash her new life and rich husband before Hilda's face.

Elliot and J.D. discover a major change in their relationship, when they spend time together looking for a former patient.

Dr. Cox contemplates his new job title, when Ted refuses to do the majority of his work, whilst Carla and Turk talk about the possibility of a second baby.

Meanwhile, the Janitor claims Dr. Kelso's abandoned office for the brain trust.

A deer is illegally killed, and the person found with the body in the back of his truck apparently dressed it in a tutu. The man, who was completely drunk when arrested, was being interrogated by Nick and Brass, and suddently tries to escape and is pepper sprayed. Unfazed by the spray, officers then taser him and he bursts into flames and dies. Meanwhile an unbalanced woman wearing a tinfoil outfit at the front desk tries to warn people (including Nick) of an alien invasion. After observing the burning man, she later turns up dead with green-tinted blood. When other people turn up with the same problem, it's up to the team to figure out why. Meanwhile, an old couple dies, apparently in their sleep... but there is evidence the cases are related.

Addison visits Seattle Grace because they need skills that only she has. Rebecca/Ava returns with news for Alex. Meredith and Derek's clinical trial experiences one or two developments.

After Stanley snaps at Michael during a meeting, Toby tries to convince him that Stanley needs to be dealt with. Dwight buys Andy's car. Pam's forced to wear old glasses instead of her contacts.

Sam and Dean search for a Crocotta that uses the phone and computer as a means to communicate with people. Its goal is to get people to commit suicide and take their soul. Dean gets some news from an unlikely source about a demon holding the contract to his soul.

OCC builds an industrial-themed bike for the Manitowoc crane group. Also, Mikey gives a lecture to a group of second graders.

Floyd stays at Liz's place overnight when his flight is canceled. Meanwhile, Jack is demoted to the 12th floor, causing him to question his future status at the company. Finally, the staff celebrates Annual Sandwich Day with mysterious sandwiches from a secret location in Brooklyn.

There's been a jewelry robbery in town, and Neela finds out that one of her patients might have had something to do with it. Pratt makes an effort to save his relationship with Bettina, and Abby worries about her marriage, sensing that Luka may have given up on it.

Kate and Juliet must learn to work together when Jack's health is seriously compromised. Something goes wrong as Sawyer, Claire, Aaron and Miles continue their trek away from Locke's camp and back to the beach.

After the latest lottery winner goes missing, the team works to figure out why.
