The Angel of Death returns, but instead of the Charmed Ones, he's there to take Leo. The sisters try to use a spell to buy Leo some more time, but it backfires, leaving Piper begging for help from both the Avatars and Elders. Meanwhile, Phoebe and Billie hunt for Billie's missing sister, and encounter a demonic bounty hunter named Burke, who keeps the heads of his victims as twisted trophies.

The mayors estate is used for Springfields Easter celebration, and in the course of the events, Homer embarrasses Marge in front of some new potential friends. To make it up, Homer decides to find her some new friends, with little luck. Marge finds some new friends on her own, Tammy and other members of The Cheery Red Tomatoes, a group of women of a certain age. This group has plans for a fund raiser; they intend to rob Mr. Burns of 1 million dollars, an amount he once promised to charity. Homer finds out about the plan and tries to stop her from doing something shell regret. Meanwhile, Lisa in search of summer opportunities decides to spend the summer in Rome. Lisa tells Principal Skinner that she speaks fluent Italian, since she really doesnt she tries to take a crash course and winds up hiring a tutor that turns out to be Milhouse.

Steve becomes a famous author, and the entire family gets caught up in his celebrity, including Stan, who becomes an unbearable "stage parent". After Steve hooks up with an agent, he abandons his family to lead a "Cribs" lifestyle.

When a fisherman finds evidence of a young girl's death washed up on a shore in New Jersey, Lilly re-opens the 1965 case of 4-year-old child who had a seemingly abusive mother.

It's a race against time as the entire morgue works to stop a series of bloody murders. A copy-cat is replicating the crimes of the Mass Pike Killer bringing Woody and Jordan face to face with the original criminal. Jordan is unsettled when the murderer begins to unravel her emotions and pin point her intimacy issues.

George stalks Bree, despite her efforts to break up with him. Lynette takes charge of office politics. Carlos is paroled from prison and returns a new man, and Gabrielle is jealous of his new 'holier-than-thou' confidante. Susan meets her birth father and tries to begin a relationship with him.

Lois discovers she has a brother named Patrick and therefore takes the family cross-country to meet him. However, it turns out that he's in a mental institution, but Lois thinks he's sane, so she checks him out to live with the Griffins. Meanwhile, Peter starts a Society for the Acceptance of Fat Men. However, the fat men of Quahog are strangled one-by-one.so the Fat Society live in terror. Who is the strangler?

Dorie Russell is waiting for quintuplets, all 5 girls. Her husband seems more concerned than she is but she is set to bring her babies to their term of 36 weeks. All the departments are on call for the quintuplets as three of them have developed problems of different kinds. Meredith last night's stand shows up at the hospital with priapism and they need to call Derek Shepherd for a consult which puts Meredith in a very embarrassing position. Alex and Derek must take care of a patient who has a tumor inducing thirst as a side effect. On the love side, Alex and Izzie are having some slight sexual problems, while Meredith goes on adding one night stands and Cristina is thrown off course by being "keyed".

When the body of a young woman is found by two children behind a dumpster, Barek and Logan learn she had recently returned from Guatemala and was suspected of being a drug mule. The investigation leads them to a noted plastic surgeon who has made a name for herself in recent years performing surgeries in Guatemala on people who would not normally be able to afford plastic surgery. As the investigation continues, Logan and Barek suspect that the doctor has a lot to hide.

Larry has his jacket borrowed without permission, but he manages to end up the bad guy. Meanwhile, his investigation into what happened to Jeff's missing dog, Oscar, leads him to suspect his Korean bookie.

Granddad gets in a fight with the old and blind Colonel Stinkmeaner. He then has a hard time coming to terms with what happened and has to suffer being taunted by Riley.
