Malcolm's family tries to become friendly with their new neighbours with the pool, but end up with new enemies instead. Neighbour boy Josh convinces Dewey that the lawn gnome wants to eat him, and spreads vicious lies about Malcolm that have the police coming to visit. Young daughter Emily likes to bite Reese every chance she can get, and Reese can't fight back because she's just a four-year-old girl. Lois takes an immediate dislike to Tina, the mother of the family, who refuses to admit that Emily has a biting problem and treats her Hispanic gardener like dirt. Only Hal bonds with new neighbour father Mike, but they have to carry on their friendship in secret. Malcolm enlists Reese's help in smearing Josh's name the way Josh did his. Hank's friends all make fun of him when he cries at a "chick flick" called The Flowers of Time. Peggy thinks Hank was crying because the movie reminded him of his relationship with Bobby. It turns out that Hank was really crying because his beloved truck is breaking down and can't be fixed. In an effort to salvage her, Hank goes to extremes, but the inevitable is just around the corner.

Robson and Hoyt tell Rev. Cloutier to back off of Schillinger. Schillinger and Beecher have their first counseling interaction session. Tidd continues to get close to Said by defending Arif in a fight. Arif goes to Said and pleads Tidd's case to accept him into their Muslim group. Busmalis requests that he and his girlfriend be married. Glynn hosts the Warden's conference and is given an update on Hughes by Querns. Glynn soon has Hughes transferred to Oz's cops unit so he can protect him. This unit is also where Johnny Basil (Mobay) is locked up. Keller's old buddy Ronnie gets close to Beecher causing Keller to get jealous. Ronnie's offered a sentence reduction in exchange for corroborating evidence of Keller's involvement in some past murders. Beecher finds this out and warns Keller, who doesn't believe him. Keller question's Ronnie about the matter and then, comes up with his own solution. Em City inmates are selected for drug testing experiment that promises to age the person quickly therefore reducing their actual time spent in prison.

When Fry eats an egg-salad sandwich from a vending machine at a gas station, he begins to have strange side-effects - he becomes stronger and smarter. Professor Farnsworth makes a diagnosis and concludes that Fry had ingested 'intelligent worms' that have set up shop in his body. Because these worms are so smart, the regular means of flushing them out will not work. The Planet Express crew must shrink themselves to microscopic form and enter Fry's body to fight off the intruders. Meanwhile Leela is left to divert Fry's attention, but finds herself more and more attracted to the new man Fry has become.
