Simon's ex-girlfriend, Georgia (guest-star Rheagan Wallace) tells Simon she's supposedly pregnant and Simon doesn't know how to tell his parents - but Georgia is not being completely honest about the whole situation. Eric, Annie and the twins are on a road trip from Ginger's house back to Glen Oak and reminisce on old times they have had together. Meanwhile, Matt rushes Lucy to the hospital after she collapses at the grocery store where they were shopping for Thanksgiving food. A frantic Matt can't track down Kevin as he is working on an undercover drug bust. Matt tries to take control of the situation when arriving at the hospital with Lucy but, Matt's semi-medical education puts Lucy more on the edge when talking to the doctor about her medication and treatment of the baby. Lastly, a lonely Ruthie is having a miserable time in Puerto Rico with Mary, Carlos and baby Charles and calls home for comfort.

Contestants take part in a thanksgiving menu that includes sheep brain pie, bile gelatin with cave spider topping, night crawler green beans, worm and beetle stuffing and maggoty mashed potatoes.

Amy and Ephram plan a romantic night together at the Abbott cabin in order to take their relationship to the next step. When they arrive at the cabin, and after their parents worrying about how they'll handle having sex, Ephram and Amy worry that their relationship won't be able to handle the next step. Back in Everwood, Andy and Amanda chaperone Delia and Charlie on a date, but the evening takes a bad turn when Andy assumes he's on a date with Amanda; and Nina gets help from Jake after Sam is hurt ice skating and her feelings towards him begin to change.

When Charlie leaves the house to avoid a date that he can't stand, he ends up at a coffee shop, where he reconnects with his old girlfriend Lisa, now a divorce with a child; Jake tries to justify to his father why he isn't doing so well in school

The team investigates modern-day piracy off the coast of Miami after five bodies are discovered floating in the ocean tied together by rope. Horatio suspects that a ship was hijacked with some of the crew killed and then dumped overboard. Evidence suggests that other crew members might have escaped on a life raft. Using radar and GPS technology, the CSIs find survivors but their stories don't match. Now, Horatio and the team must learn what really happened out in the ocean as they find themselves investigating the seamy underbelly of Miami's militias and arms dealers.

On the anniversary of his mother's death Ichigo and his family visit her grave. While he is there Ichigo remembers the day his mother died. Rukia is found by a Shinigami who asks her why she hasn't returned to the soul society. Ichigo accidentally tells him that he is her substitute Shinigami; which happens to be a felony. Meanwhile Ichigo's sisters are being attacked by a Hollow.
